Dear Editor:

I am writing this letter to you to assist the general public in locating my Family Medicine Practice. I have been practicing as a family physician since the spring of 2022 in association with the Fresh Start Health Center in Grayson Kentucky. Until the first of August 2023,  I served as the primary care Physician resource in collaboration with Vanessa Flannery, nurse practitioner/DNP whose principle role has been to provide Medical assistance for those patients with an addiction disorder.

As of August 1, 2023 a new nurse practitioner has been hired to perform the primary care duties. Upon learning of this change, I ask the clinic owners to advise the patient population of this change and to allow those patients who wished to continue in my care, be allowed to do so through telehealth visitations or direct patient care at the Fresh Start Health Center in Ashland, Ky. To my knowledge, that community-based notification has not happened.

As residents of appalachia you know when an established physician suddenly becomes unavailable, it is usually due to a legal problem with the practice or physician or an unexpected illness that takes the physician out. Please let me assure you, my patients and the leadership of the Carter county communities that to my knowledge, I am NOT being relocated for any legal issues and I am not ill.

I remain fully credentialled as a board certified family physician with over 30 years of service in the US Air Force, private practice and as an employed physician in deployed military settings, office-based and hospital-based settings. I am available to care for any patient who wishes to see me in Ashland, Ky. My office hours are from 9am to 5pm, Monday through Friday and I am available for telehealth visits whenever appropriate. Also, experienced nurse practioners work side by side with me along with peer support and case management experts. We strive to meet your addiction recovery needs at one medical home; always seeking total recovery, optimal quality of life and sustainable good health.

Our Clinic is located at 336 29th St; Suite 203,  Ashland, Ky; 606-225-8200, ext #1.

Respectfully yours,

Jon H Walz, D.O.
